Get Started with Medtriq

The first step for new patients is to complete the appropriate intake form. This helps our team understand what kind of care you are looking for and how to route your request.

Medtriq is telehealth-first across Washington State. In many cases, evaluation and follow-up can happen virtually from a private location that works for you. Local patients may also have access to the University Place office when appropriate.

Use this form if you are seeking mental health evaluation or medication management for concerns such as depression, anxiety, panic, PTSD, ADHD, bipolar disorder, OCD, insomnia, adjustment disorders, or personality-related concerns.

What happens after intake?

After you submit your intake form, Medtriq reviews your information and helps determine the next appropriate step. Appointment options may be displayed or offered after intake completion and review.

Completing an intake form helps Medtriq understand what kind of care you are seeking and how to route your request. The next step depends on your needs, provider availability, appointment capacity, and clinical fit.

Because Medtriq is virtual-first, many patients are able to start care without needing to travel to a clinic for every visit.
